The INTERROGATIVE FORM of the SIMPLE PAST TENSE, follows this structure:

     'Did + subject + infinitive + rest of question ?':

      Did           I           walk         yesterday          ?

      Did        she          play         with her brother?

      Did        they         drink       your water         ?

Answers: Yes, + subject + did  /  No, + subject + didn't

EXAMPLE:

  Did she walk to the shop? Yes, she did /No, she didn't

  Did they live in Spain? Yes, they did / No, they didn't

** NEVER WRITE 'Did + subject + simple past'

                          DID   he  played  last Monday?  X

                          Did   he   play     last Monday? 

WH-QUESTIONS IN THE SIMPLE PAST

These questions require more information in their answers. Typical wh- words are what, where, when, why, who, how, how many, how much. 

  'wh-word + did +   subject   + infinitive +  rest of the question'

   When      did       you              buy            that sweater?

      Why      did   the computer   break down?

      How      did    the students    study         for the test?  

      How      did    the students    studied       for the test?  X
